This Questionnaire is aimed at helping you to be comfortable at work and to identify areas where some further guidance might help you to make appropriate adjustments to your work environment. It will also help to identify issues that might need to be addressed and provide recommendations to achieve this.

The content of this Questionnaire will be treated confidentially but will be shared with your employer to allow them to meet the requirements of the DSE Regulations.
